The application deadline for the 2009-2010 school year was 5:00 pm, May 1, 2009.
Early submission of your application is strongly advised. Applications received incomplete, missing information, or after the deadline will not be considered. If you would like confirmation of our receipt, please enclose a self-addressed, stamped postcard or envelope.
Applications must include:
- A completed, signed Scholarship Application Form.
- Copies of all pages of your 2008 IRS Form 1040 (including all schedules) and copies of all 2008 W2 and/or 1099 forms for individuals listed in Sections 1 and 2. (If you have not filed or will not file a tax return, see the information specified on page 2 of the application under “Household Income.”)
- A $15 non-refundable application processing fee made payable to The Independent Institute. (Returned checks will incur an additional $20 bank fee. Results from the application will not be released until after payment has been made.)
- Two letters of recommendation for each individual child applying for the scholarship describing how the child will benefit from a scholarship and/or a private education. Only one child may be represented by each letter (i.e. if you apply for two children, you must submit four letters). Letters must be written by a non-relative (e.g., teacher, counselor, religious or other activity leader) and include the writers telephone number.
- Recent academic report for each student: including a) School transcript; OR b) the two most recent report cards (or the two most recent progress reports) from the school currently being attendend.
